Withdrawal Checklist

Students planning on officially withdrawing from Appalachian State University need to make sure that several different departments across campus are properly notified.  In an attempt to help students with this process, a checklist of necessary steps has been provided below.  Downloadable copies of this checklist are available at the bottom of this page.
 

WHAT DO I NEED TO DO TO WITHDRAW FROM ASU?Students during class

Step One: Withdrawal

  •  Regular Withdrawals: Contact the Registrar's Office to sign appropriate form.  For questions, contact Martha Wilson - (828)262-2050.
  • Medical Withdrawals: Meet with Health Services professional.  For questions, contact Norma Adisey - (828)262-3100.
  • Psychological Withdrawals: Meet with Counseling Center professional.  For questions, contact either Carol O'Saben or Dan Jones - (828)262-3180.

Step Two: If You Are Living On-Campus

  • Within 24 hours after your completed withdrawal you must check out of your residence hall.  This includes meeting with a Resident Assistant (RA) or Residence Director (RD) to complete  your check out and return your key.  For questions, contact your Residence Director or the Department of Housing & Residence Life at (828)262-2160.
  • If you plan to return the following semester and are interested in living on-campus, be certain to contact the Department of Housing & Residence Life at (828)262-2160 to inform the Housing Assignments area of your intentions.

Step Three: If You Have Financial Aid

  • Contact your Financial Aid counselor and explain your withdrawal from Appalachian State University.  For questions, contact Kay Stroud - (828)262-8687.

Step Four: Books

  • All rental books must be returned to the University Bookstore.  For questions, contact Cathy Marks - (828)262-3070.
  • All library books must be returned to the Carol Grotnes Belk Library & Information Commons.  For questions, contact Jack Love - (828)262-6904.

Step Five: Health Insurance

  • Make sure your insurance coverage will continue while withdrawn from the university.  For questions, contact your local insurance representative.

Step Six: ASU Post Office

  • Go by the ASU Post Office during regular business hours and complete a change of address form.

Step Seven: Questions
